Develop the IEP

Develop IEP goals to prepare students for college success with a focus on planning for future education. The first step should start early and continue throughout high school 9th, 10th, and 11th grade.  


Work-Based Learning

Explore work-based learning and job specific skills through in-person employment and skills assessments. The second step should start around age 16, ideally 11th and 12th grade. 

College Transition

Discover college and community program options with a comprehensive discussion on diploma pathways, actionable steps, and funding options for college. The final step should start before 11th grade and continue through 12th grade. 

  

Secondary Transition Planning

This service is focused on Postsecondary Transition Services for students transition from high school to assist families in understanding the different diploma pathways, transition services, goals, work-based program options, and help prepare for college program success. 


As defined in IDEA, transition services are a coordinated set of activities for students with disabilities that is designed to be results-oriented that focuses on improving the academic and functional achievement of the student with a disability to facilitate the student’s movement from school to post-school activities, including postsecondary education, vocational education, integrated employment (including supported employment), continuing and adult education, adult services, independent living, or community participation.

Successful transitions are well-planned, thoughtful actions designed to accomplish outcomes. The goal of this service is to provide support and advocacy across High School to supplement other advocacy efforts for students pursuing college programs.
